Introduction and Objectives of the Project
Process Failure Risk Analysis, PFRA is a Team Oriented Problem Solving (TOPS) method, aimed at minimizing dissatisfaction, and financial loss for process related failure risks. This method will be applied during the planning stages of a process, and then updated on a regular basis to document changes.
This PFRA tool describes a systematic, rule-based, quantitative, and open-source computational model developed to identify and minimize potential failure risks, and/or failures of processes in general. It is aimed at minimizing dissatisfaction, and financial loss for process related failure risks. In other words, PFRA addresses negative quality and is concerned with potential events that can make any process fail. PFRA is applied during the planning stages of a process, and then updated on a regular basis to document changes.

Introduction and Objectives of the Project

A Brief Description of the Methodologies Applied
There are seven key aspects of our generic methodology one should respect and follow, before beginning to work on any process failure risk study with our PFRA Tool
The seven key aspects of this methodology one should follow are:
- Define scope, function and purpose: define the system or sub-system, or any product, or process design to be analyzed;
- Identify potential risks and failure modes, and their effects:gradually deepen the product, process or system design analysis and locate all potential failures at the earliest possible stage;
- Prioritize potential failures: all faults should be ranked, located and eliminated as early as possible;
- Select and manage subsequent actions: in cases in which the risk of potential failures is high, an immediate control plan must be devised and subsequent actions should be taken to control the situation;
- Observe and learn: PFRA represents a dynamic, living knowledge-managed document that needs to be kept up-to-date;
- Document the process: store the PFRA contained information in a widely accessible format for the current as well as for future Teams;
- Enter the values into the PFRA spreadsheet on a relative scale: values are determined by the local PFRA Team, as well as by the local, customized standards.
- Severity: a rating corresponding to the seriousness of the effect(s) of a potential failure mode (the value range is between 1 and 10, where 1 is least severe, and 10 is most severe);
- Detection: a rating corresponding to the likelihood that the currently executed process controls will detect the failure mode (a rating value of 1 to 10, where 1 is assigned if it is ‘almost certain’ that the failure mode will be detected, and 10, if it is ‘almost impossible’ that the process controls will detect the failure mod);
- Occurrence: a rating corresponding to the rate at which a first level cause and its resultant failure mode will occur under currently employed process controls (rating has a scale of 1 to 10, where 1 means ‘almost impossible’ and 10 is ‘almost certain’ to occur).
The Main Body of The Project
Based on the model process from CIMpgr and requirements of customer from one of collaborative companies, Pentax Ricoh, that BGRM and PR have agreement to develop and innovate to launch product that made from recycled plastic. The problem of the quality of product, camera body will be applied in the PFRA spreadsheet.

Regarding to the BGRM-PFRA-Spreadsheet, from the testing data collected by Quality Assuarannce Team, the highest risk score of the Accumulated RPN is on Process ID12: Examine camera waterproof, the RPN score is 233.28 points. That can make camera broken and damages electronic circuit boards and processing inside cameras. The severity rating is 8 which means that due to the process performed, there is, an Extreme Effect on performance, or process operation in that the device fails to start, or operate. The internal and/or external customer will be severely effected, system will not be operable, but will be considered to be safe. The detection score is 6, which means Detection of Failure is Low on the part/ component, and/or on system performance, and/or on subsequent process, or operation. In other words, there is a low likelihood, that the current controls in place will safeguard against process failure. The Occurrence Rating is 3, which means Occurrence is Very Slight on the part/ component, and/or on system performance, and/or on subsequent process, or operation. very few failures likely. From these three rating factors made the RPN score was 162, but the score was raised by the tooling factor for 1.2 and the customer standard factor for 1.2, eventually the final score, based on all factors that were mentioned above, reaching to 233.28 point.
In this case the risk associated value is high, over the high standard 200, that means it needs to take action immediately. The next process to decrease the risk is to analyze the camera material that made from the recycling plastic and detect the error from production line.
The second highest RPN score is Process ID9, Examine battery slot opening system, that score 60 point. However, the RPN value is considered as low, so we won’t take action to this process but it still needs to continuously observe.
Collaboration with 4 Partner Companies
PENTAX A RICOH COMPANY: Since this project is focusing on the quality of camera body that directly relates to Pentax. On this project the Pentax are going to help our R&D and Quality Assurance Team to detect the defect and determine the customer quality standards.
SONY HANDYCAM: On this project, Sony will come to observe the operating line and R&D department. In order to the next project will be the video recorder body maded from recycled plastic. To study the positive and negative effects of the waste plastic recycled product will help Sony to shorten the duration time of manufacturing and developing the product.
SEW EURODRIVE: This company helped BGRM to develop and redesign the plastic processing machines since a large number of experienced engineers from SEW Eurodrive were sent to work with BGRM engineering team to detect the gas emission and solving the problem of the plastic melting.
ASSOCIATION FOR MANUFACTURING TECHNOLOGY (AMT): AMT are widely known in the field of engineering and technician, so this company provided the list of company to supply NDIR sensors to detect the gas emission problem.
Sustainable Greening:
All of main products and processes from Blue in Green Recycling and Manufacturing company relate to the green engineering and sustainable technology. The first benefit from sustainable greening is to sustain the environment and ecosystem that will reduce the waste plastic then decrease the air pollution that will be generated from plastic burning. Another benefit is to make the brand reputation of BGRM company since, currently, most people tend to consume the product that does not create waste and can be recycled. From this reason, it will make the company has a good reputation for the customer and a lot of long experience and technology advanced companies want to be partners. The last benefit is cost saving, BGRM plan to build the renewable energy power plant to supply operating system that will save electricity cost in long term since, the electric energy price tends to increase continuously in the future.
Video Reviews:
a. I-beam Roller Quality: Focusing on the roller, one of the wheel in the roller does not spinning that creates fiction between the roller and I-beam. The risky from the low quality product is caused by the poor quality manufacturing control and cheap product design. The fiction from the rollers and I-beam will make the movement unstable and make the process are not sustainable. Another point is the product manual, as you can see in the video, the product manual is very dirty that illustrate the products is made from the company that doesn't not care about the package quality, it also shows that this company has bad environment.
b. Marc-Bauman-Interview: Looking at the different approaches to floor mat design used by both companies reveal different risks. There is a risk that the floor mat could end up on top of the accelerator due to pedal height and design flaws that do not keep the mats in place. The design, in an effort to ensure floor mats cannot end up on top of pedals, have the risk that they may not cover enough of the floor to provide protection from wear and dirt. If choosing between customer safety and carpet replacement, though, the Toyota design has a bigger risk to deal with.
c. MINI Quality VipodLx: The most obvious risky process in this video is the way they shred cabbage. They risk of cutting their fingers, getting cigarette ash in food, and handling the shredded cabbage with bare, unwashed hands. The omelet-making machine, although very interesting, also shows some process risks. The machine looks like it makes about 360 omelets an hour, but the leftover raw egg cannot be easily cleaned, which could lead to health risks.
d. Not Green Generator Challenges: First, as you can see from the video, the generator is making the loud noise that might disturb the people around that area. Another problem is about the safety, the wire to conduct the electric power to the room at the resort is not safe the protective cover does not cover a whole of system. It is very dangerous if water leak or come inside the pipe. However, this location is suitable for setting the renewable generator both solar plant or wind turbine.
e. OKUMA LeanCells VopodLx: This video showed how informal way of communication with employees can be more productive than the formal one. However, this has to be applied with the caution, especially when it comes to safety. Safety is the area in which established rules need to be followed, and if you are giving a lot of freedom to your employees, they can intentionally or unintentionally to disobey some safety measures during the boat-trips. Also, Sam asks of his guides to give instructions to customer how to behave environmental-friendly to Jellyfish Lake, but his own boats can be dangerous for the Lake.
f. Palua M2012 Long Lake Kayaking Tour: This video encourage me to make a summer trip for kayaking and sea diving. Nevertheless, first thing that is not sustainable that is grilling sausage and chicken by charcoal. The charcoal is made from carbon and burning the charcoal will pollute the environment around there. Another point is noise, even the speedboat is convenient for traveling with few people but, it makes high noise that might disturb the fish and sea life. So, in my opinion I think that it would be better if we can reduce the noise engine from speedboat.
g. PeterRayson Intell Enterprise: This video is the interview of Peter T Rayson, head of Design and Process Innovation at Technology Innovation Center UCE Birmingham. He was talking about the management of the project and giving his opinion how the company need to do to sustain the company. One thing that very important that the company has to keep moving to make innovation and improvement its process and/or product. If the company stops to develop itself, finally, it will be replaced by the other company.
h. RAMPF Robotic Sealing VipodLx: The robotic production machine works faster and more accurate that man power, this technology will save a lot of cost in the future. However, for the unsustainable case, since the robotic machine works very fast it can replace 200-1000 labors, when the engineer making wrong design or the robot has defect about the controller program it will cost more from effect of mass production. Moreover, this machine still needs the expert to detect or and to fix when it's not work.
Panorama Navigation Map:
Applying to PFRA tool, the Panorama Navigation Map can be used to illustrate the risky in each process step by drawing or taking photo of each process ID. I believe that it will help the reader to understand clearly and easily.
This technology enables 360 Degree view of the scene using images, video to be projected in panorama view, which makes it an excellent learning tool. The small city that was illustrated and described in panorama term e-book making the reader feels that he/she is traveling in this town.
After get the standard requirements from collaborative companies, Sony Handycam and Pentax Ricoh, the PFRA will be applied in to the image in Panorama Navigation Map. Then, we will use the audio file to explain the details in each process that what is the risky aspect and what is the effect of them, same as the professor did to explain the small town. The RPN scores, combining with Severity Rating, Detection Rating and Occurrence Rating will pop-up when the reader pointing mouse to each process ID in the table format that will be easier to see and understand. Another picture that will be used to applied in the map is the chart and the factors that help the reader to compare the difference between each process ID that which one should be the most considerable or which process needs to improve abruptly. And, the next step plan will be explained in each process to provide the idea to the reader that what should he/she should do before making decisions in next step.
Finally, I think that the Panoramic Navigation Map can make people understand the PFRA clearly, and it will reduce the complicated because the large size of spreadsheet might make reader spend long time to understand each process. The new type of map can reduce the image to make it is more compatible.
Further Work Needed/ Proposed:
To control the final product toward to satisfy the customers, it is extremely necessary to continuously detect and measure the quality of product and operating efficiency. The tool and the process that were provided from the ebook will be helpful to control the process.

This article talked about the NZET project, Zero Net Energy Towns, in North South Wales, Australia. The author discussed the possibilities of the project, and how Adam Blakester has worked to pursue his effort. In my opinion, the main factors to make the project succeed are the sources of renewable energy such as bio-gas, solar, wind and hydro generation. The town that has potential to be NZET should have various kind of energy source to distribute risk, the amount of sources is also important to generate energy. The other important thing is supporting from government, since the budget of building the renewable energy plants is much more than building the general plants. However, it will be worth in the future because the cost of energy tends to increase continuously.

In this article, the author showed the readers how the NV Energy is success even it faced a lot of problem of promoting EVs in Neveda, then he suggested the other utilities to follow the NV Energy. The EVs rate has grown increasringly in this few years, since NV Enegy and Neveda State Government put more effort to promote the EVs car, such as increasing the charging stations in the most common visited places, and solve the problem of high peak load in summer. In my opinion, I greed that high peak demand of anargy can create problem and to use the EV car is a smart way to invest because the cost of energy will be so much cheaper than the cost of fuel in the next decade.

The auction project for 500MW solar power plant has been set to solve the problem of enegy shortage in India, to build the 500MW plant in ten months is very short period but there is high demand from India population that the government need to do it abruptly. In my view, I believe that the amout of the population of India will increase continouosly. So, the Idian government is doing the right way to solve the problem by creating more energy plants and the smart thing is they chose to build the plant that use renewable source. But, the government need to control the contractors carefully because it seem a lot of its budget will be spent for this project.

I believe that I deserve the point 10/10, in order to completing all requirements from the assignment direction and creating the process models that meets the assignment's objectives.

The auction project for 500MW solar power plant has been set to solve the problem of enegy shortage in India, to build the 500MW plant in ten months is very short period but there is high demand from India population that the government need to do it abruptly. In my view, I believe that the amout of the population of India will increase continouosly. So, the Idian government is doing the right way to solve the problem by creating more energy plants and the smart thing is they chose to build the plant that use renewable source. But, the government need to control the contractors carefully because it seem a lot of its budget will be spent for this project.
